Warren, Mich. – June 29, 2018 – General Motors has recalled an estimated 3,233 vehicles after a recent update to the Hybrid Powertrain Control Module 2 could potentially disable the cell balancing function and result in a low-voltage condition. The recall affects select 2013 Chevrolet Volts, according to documents from the National Highway Transportation Safety Administration. The estimated figure of cars with this particular defect is 2 percent.

The Defect

Vehicles that received a software updated in a service procedure from a dealer may contain an error in the software update that could prevent the batteries from balancing the voltage in singular battery cells. Under certain circumstances, this may result in a low-voltage condition in one or multiple battery cells. The vehicle will notify drivers if the battery cells fall below a certain level and if the driver continues on the road after entering reduced power mode, the vehicle may lose propulsion. This incident may increase the risk of crash and injury.

Timeline of Events

A GM engineer sent in a Speak Up For Safety report on May 11 after finding that a software update used on select 2013 model-year Chevrolet Volt vehicles contained an error that could potentially prevent batteries in some of these vehicles from effectively balancing the voltage among individual battery cells, according to internal reports. Following this report’s submission, GM launched an investigation, where the investigator reviewed field data, which demonstrated elevated rates of the condition in the field for vehicles with this update.

Afterward, GM’s Open Investigation Review board reviewed the investigation June 6, and the GM’s Safety and Field Action Decision Authority ultimately decided further action was necessary and issued a safety recall for these vehicles on June 7.

GM notified dealers of this safety recall June 14 and the National Highway Traffic Safety Administration June 20. Owners will receive recall details and dates will available.

The Solution

According to an internal communication, GM will notify drivers, while dealers will reprogram the Vehicle Interface Control Module module at no cost to the cars’ owners. A notification schedule has not yet been provided, but customers who own this vehicle will be notified by mail or can check their Vehicle Identification Number on GM’s website. Owners who want further information can contact Chevrolet customer service at (800)-222-1020. Warren, Mich. – June 27, 2018 – General Motors has recalled almost 500 vehicles potentially equipped with defective engine software, according to documentation submitted to the National Highway Transportation Safety Administration. The campaign affects 2019 Chevrolet Corvette ZR1 sports cars manufactured between Dec. 8, 2017, and May 31, 2018. The American automakers suspects just 1 percent of the models involved in the action contain the defective software.

The Defect

Affected vehicles come with sensing diagnostic module software that may fail when exposed to hard braking or extreme acceleration. In the event of SDM failure, crash sensing and air bag deployment functionality may be inhibited. This defect therefore poses a serious safety risk to occupants. However, GM has yet to receive field reports connecting the defective software to any accidents or injuries.

Owners in possession of vehicles that have experience SDM failure will not receive airbag readiness or diver information center notifications.

Timeline of Events

GM personnel on April 30, 2018, hosted a media demonstration in Atlanta, during which a 2019 Chevrolet Corvette ZR1 was subjected to race-like driving conditions. Following the event, GM engineers assessed the vehicle and found that its SDM software was not operating properly. These technical specialists brought the issue to the attention of the automaker May 3, 2018, via an internal Speak Up For Safety report. GM opened an investigation in response May 8, 2018.

During this inquiry, the engineering team at the GM Proving Grounds in Milford, Michigan conducted road tests involving hard braking and extreme acceleration. Following these trials, the group reviewed vehicle performance data and concluded that such maneuvers caused SDM overstress, which in turn led to complete software failure.

The GM Open Investigation Review evaluated the the matter May 21, 2018, before sending it to the Safety Field Action Decision Authority. The SFADA ultimately decided that a voluntary recall was necessary and initiated such action May 24, 2018.

GM notified dealers of the forthcoming campaign May 31, 2018.

The Solution

The automaker has directed dealers to reprogram the SDM software in affected models free of charge, according to an internal communication. Warren, Mich. – June 25, 2018 – General Motors LLC has recalled several thousand of its Chevrolet Volt models due to a software update flaw that may adversely affect the plug-in vehicle’s battery functionality, according to documentation submitted to the National Highway Traffic Safety Administration. The automaker believes the recall is warranted because affected cars may result in a crash, especially in situations where drivers need to accelerate to keep pace with the flow of traffic.

The Defect

Earlier this month, GM recalled approximately 3,233 Chevrolet Volts – with 2013 model years – after dealers performed a software update of the plug-in hybrid’s on-board battery, specifically its Hybrid Powertrain Control Module 2 or HPCM2. The update was meant to improve the life and sustainability of the HPCM2, but some of the software updates were corrupted, the company believes, potentially resulting in a voltage imbalance situation between the individual cells that give the plug-in hybrid the energy it needs to operate. Should the voltage dip below a certain threshold, the vehicle may enter a reduced power mode, thus preventing motorists from accelerating.

Although General Motors suspects just 2 percent of the 3,200+ Volts are affected, the automaker contends the recall is warranted because motorists operating the Volts with the flaw are at an increased risk for a crash due to a loss of propulsion that impacted vehicles may experience. No known crashes, injuries or deaths have been linked to the improper cell balancing defect.

Timeline of Events

The issue first came to light in May, when a GM engineer discovered the problematic software and informed GM of the issue after filing a Speak Up for Safety report, based on a chronology of events submitted to and published by the NHTSA. GM subsequently launched an investigation, although the automaker was already looking into a similar but unrelated issue relating to low-voltage output. The investigation revealed that the likelihood of Volts losing propulsion seemed to depend upon whether the vehicles had received a software update.

By June 7, GM’s Safety and Field Action Decision Authority determined that a safety recall was appropriate out of an abundance of caution.

The Solution

GM says it will inform owners of 2013 Volts about the software glitch, who can then go to their nearest dealership to have their HPCM2 reprogrammed for free, according to an NHTSA internal briefing document. The reprogramming will entail an update to the Volt’s vehicle interface control modules. However, GM didn’t say when the notifications would be sent out or how motorists will be informed. All of the affected Volts were sold within the continental U.S.

General Motors has recalled a number of sport utility vehicles believed to be equipped with defective drive shafts, according to documentation filed with the National Highway Transportation Safety Administration. The campaign affects 2017-2018 GMC Arcadia, 2018 GMC Terrain and 2018 Chevrolet Equinox models. The automaker estimates that 3 percent of the vehicles have problematic drive shafts.
The Defect
Models referenced in the recall could contain front right intermediate drive shaft assemblies prone to fracture and separation. Engineers have traced the issue to the parts manufacturer Nexteer, which failed to use properly calibrated ultrasonic scanning equipment when performing quality checks.
The defect poses a serious safety hazard to occupants, as vehicles with damaged drive shafts may lose power during operation or roll away when stopped without the parking brake engaged. However, GM has yet to receive reports of accidents or injuries connected to the defect…

GM Recalls Sports Cars Due to Air Bag Defect

Warren, M.I. – Feb. 21, 2017 – General Motors Co. has recalled more than 91,000 Pontiac and Saturn sports cars with defective air bags. The recall affects Pontiac Solstice coupes and convertibles, as well as Saturn Sky roadsters produced between 2005 and 2009, according to documentation from the National Highway Transportation Safety Administration.

The Defect

Both vehicles are equipped with passenger air bag suppression systems that leverage seat sensors to determine if passenger safety features should be deployed in the event of a crash. The sensors used in these systems can bend and become damaged over time with repeated passenger contact. In some cases, these devices can malfunction completely and inhibit air bag deployment, even when occupants are present. This presents serious safety risks, as passengers could be left without protection when an accident occurs.

Timeline of Events

In April 2016, an owner submitted a defect petition requesting that GM investigate the PPS systems in Solstice and Sky models. Roughly one month later, the automaker opened an internal inquiry. The NHTSA made GM aware of additional complaints soon after and, in July, solicited warranty information for potentially vehicles. On Dec. 15, GM launched an official investigation. Engineers met with NHTSA officials throughout December and January 2017. The automaker’s Safety Field Action Decision Authority initiated a recall Jan. 19 and notified dealers Jan. 26.

The Solution

GM has not yet developed a workable solution to the problem, according to documents filed with the NHTSA. There is no timetable for when a solution might be available but dealers have been advised to address customer concerns. Consequently, the automaker has updated its Global Warranty Management system so dealership personnel can effectively respond to individuals who may own affected vehicles. GM also advises Solstice and Sky owners to contact customer service for additional assistance.

Warren, Mich. — September 27, 2016 — In recent GM recall news, some 2016 Buick Verano autos may be subject to a fire hazard due to fuel line defects. The Part 573 Safety Recall Report from the National Highway Traffic Safety Administration website stated that affected autos have a 2.0 liter turbocharged engine and were manufactured between Feb. 16 and May 12, 2016.

This source said that fuel lines in these Buicks could fail if they make contact with heated sections of the engine. This could lead to a fire if the lines in question are worn away enough, causing fuel to leak out. A total of 189 vehicles are potentially involved in this action, and all 100 percent of them are estimated to have the defect.

The first engine fire associated with this problem occurred June 20 in the car of a GM employee, according to the Report. As of Aug. 10, this was the only fire caused by this defect that had been seen in the field.

Dealer Notification was planned for Aug. 10, and the remedy requires the dealer to replace the fuel line assembly entirely. Since the defect comes from improperly assembled fuel lines, the new replacement assemblies will have the proper engine compartment routing. GM found out that the vehicle issue stemmed from “poor operator training” on July 18.

A recall bulletin from the manufacturer listed all of the parts needed to complete the recall’s system replacement. This list includes eight parts total, such as a fuel-feed pipe and charge air cooler inlet air tube adapter seal for each vehicle.

Chance of Fire Prompts General Motors Recalls

Warren, Mich. — September 13, 2016 — Wiring issues may make a fire more likely to break out in one Buick model. Although there is no remedy scheduled as of this writing, one of the more recent General Motors recalls addresses this problem. The component manufacturer in this recall is Lear Corporation, based in Germany.

The official Part 573 Safety Recall Report on the National Highway Traffic Safety Administration website said that the recall potentially involves 42,984 2011 Buick Regals. A full 100 percent of these are thought to have a wiring harness problem that could wear down the wires themselves.

This chafing could potentially lead to a short circuit, which in turn could start a fire within the vehicle. In addition, the report cited other possible side effects, such as smoke, sparks and flickering instrument-panel lights. Despite this, it did not list any signs to be taken as “identification” of the defect.

The fix requires dealers to repair faulty wiring, if needed, after inspecting and securing the harness. There is also no apparent danger of wire chafing in similar vehicles manufactured after 2011 in Russelheim.

Though the decision to conduct the recall only came June 29, it dates back to Dec. 21, 2015. On that date, the source said, customer reports of fire led to a a Speak Up for Safety report.

A July 7 notice for dealers said that the parts necessary to conduct this recall fix were not currently available: dealers can only proceed once they are. This statement also announced that the Global Warranty Management system would update to reflect the new recall, as a way for dealers to prepare to address owner concerns.

Welding Issue Triggers GM Recall

Warren, MI — January 13, 2016 — Three different models of 2016 vehicles are included in one of the more recent GM recalls, with a manufacturer’s report date of December 9. This action concerns 2,633 potentially affected vehicle models, all of which could include an improperly welded seat that leaves occupants in danger during a crash. The vehicles have faulty third row lower seat frame welding as a result of problems with the robot welder at the supplier, Michigan-based Bridgewater Interiors.

Just 1 percent of the vehicle population was estimated to have this defect.

The issue was initially discovered November 2015, the same month in which the recalled vehicles were produced. According to a safety recall report also available from the organization, the welder working on the vehicles had a “defective servo motor.” Since November 2012, newly produced versions of these models have been welded by robots with the correct motors.

Dealers were notified on December 9, and just 1 percent of the vehicle population was estimated to have this defect. The report explained the actions dealers were instructed to take to correct this problem.

“Dealers will inspect the third row left side lower seat frame and, if necessary, replace the lower seat frame,” the document stated. “Vehicles built during the affected build date were inspected to identify and replace seat frames built by the defective robot welder,” it added.

In an instructional recall bulletin, General Motors noted that “only approximately 11 involved vehicles” would require a seat frame replacement. As such, it emphasized that dealers order parts only as they need them, to make the most out of the limited replacements in stock.

New ignition switch problem leads to GM recall [Video]

A new GM recall concerns ignition switch problems in some Chevrolet Silverado and Suburban vehicles, as well as GMC Sierras and Tahoe SUVs. Fortunately, the problem appears to have been detected early on, with only five accidental shutoffs attributed to it and just over 3,000 autos recalled. In affected vehicles, the cars can get stuck in the ignition when interior temperatures are high.

The lock cylinder in these cars could accidentally be shifted into “accessory” mode, making the truck less safe to drive and leaving it more defenseless during crashes. There have been no injuries reported yet connected to these issues, and is supposedly unrelated to a previous GM ignition switch recall.

The remedy for this issue involves changing out the housing around the ignition area.

Airbag concerns lead to new GM recall

Washington, D.C. — August 25, 2015 — Adding to the recent wealth of recalls from various manufacturers over airbag deployment concerns, General Motors is now recalling thousands of Chevrolet Impalas. This issue focuses on a defect that would leave only infant passengers and younger children at risk, according to the Detroit News. Owners with children may have to think carefully about the possible dangers of driving these cars with them in the front seat.

Impalas manufactured as late as this June could sport a defect that doesn’t detect if a child seat is installed in the front of the vehicle. The error comes from a calibration problem in an electronic vehicle module, which in turn traces back to issues during vehicle assembly. 

The National Highway Traffic Safety Administration (NHTSA) features a recall bulletin detailing the steps needed to correct the “passenger service system” in affected vehicles and improve safety. The fix reportedly takes less than an hour of labor time. The recall specifically applies to those vehicles with vented-heated seats.

“Before the passenger presence system can be re-zeroed the front passenger seat must be completely empty of all items,” the bulletin reads. “The presence of any liquid or items on the front passenger seat will affect the calibration and operation of the passenger presence system.”

A total of 8,989 Impalas are targeted in this action, most of them in the United States and Canada. The NHTSA document indicates that the warning light could turn on when the seat isn’t occupied in the recalls, possibly leading users to discern exactly how at risk they and their families are.
